It is when the word been is used incorrectly instead of being, for example:

I was been really nice to them

It really, really annoys me. Irrationally so. My grammar is nothing to write home about, but at least i attempt to use the English language and its words in the right place (I may hyper ventilate about Pacific and specific in a moment too).
