Could a German native speaker help me with a bank question?

I live in Hannover and we're moving back to UK.I need to get the dc's savings from Berliner Sparkasse where we used to live and somehow left the savings there, this is what the woman wrote to me:

'...können sie uns auch formlos einen Antrag und die Sparkassenbücher zur Auflösung und Übertragung auf Ihr Girokonto bei der Berliner Sparkasse senden. Der Antrag muß von beiden Elternteilen unterschrieben sein.'

So anyone have a clue what I should write in this formlos thing?

No a native speaker but I would think they man send your books to them and both parents must sign te form?

Not a German native speaker - but the message says 'could you also send us an informal request and the savings books so your account can be cancelled and tranferred. Both parents should sign the request.'

Hiermit Bitte ich Sie, meine Sparkassenbücher schnellstmöglich aufzulösen und den Restbetrag auf mein Girokonto zu überweisen

or formlos
Ich möchte meine Sparbücher auflösen.
Bitte überweisen Sie mir den Restbetrag auf mein Girokonto.
