CCG1 and CCG2 forms HELP please

I am filling out CCG1 and CCG2 forms.
They need to be sent off to Student Finance Co by Friday.

I'm applying for help with my child care payments made 01/09/2008 to 26/10/2008

and for 27/10/2007 to 25/01/09

PLEASE help me I have no idea how to fill them out, do I need to do both as the CCG1 is more of an estimated form to be done at beginning of term and I am giving them accurate figures on the CCG2 form -

Any help gratefully appreciated.

Plus, it says I have to supply weekly child care costs, how do I work those out, I have no idea

erm, i usually send the ccg1 in september as they are estimates.
ccg2 at the specified times throughoout the year.
speak to your nursery cos i fill in my details and they fill in the money part. definately done that way on ccg2 as it is in their section not mine.
ccg1 if i remember rightly i was asked to do that but asked nursery to work it out for me (lazy emoticon) and they were more than happy to do this
