I can see a few posts on this topic about little ones who are trained and then regress - exactly what my DD (3.6) is doing! Used to be fine, now we have regular accidents. Please post here if you have experienced this and have come through the other side. This seems to be going on forever!

Had a few episodes of this past initial training - key thing seems to be getting on top of it straight away. At various ages they lapse because they're getting into different things, and even because they're learning to concentrate on what they're diong (hooray!) and not notice the signs that they need the loo (boo! hiss!). Have you gone back to the checking every 30 mins whether DD needs a wee? (Assuming it's wee that's the prob). We still have to practically order DS (4.9) to go to the loo sometimes, when he's doing his special wee dance and doesn't want to take time out from whatever else is going on. But with star charts and the very occasional time-out he now realises that it's not acceptable to lapse, and touch wood we haven't had a problem since the appalling fortnight when he started school . And before anyone flames me about the time-outs he'd demonstrated that he was completely capable of being continent, and that was for flagrant ignoring us when we told him to go to the loo and he ignored us then wet himself...
