wanting to use cloth nappies for first baby but need advice on how to manage this when out and about. Do I need a bigger changing bag and what are your recommendations?

has to be able to attach to pushchair, not one of the gorgeous girly handbag ones so DH will be happy to use it, and hold everything I need, including I guess something to keep wet nappies in?

You don't need anything particularly bigger. (god knows - nappy bags are big enough already!!) You will need a wet bag like these for example.

I only carry 2 clean nappies with me, and have a couple of all in ones for this purpose, so I don't need to carry a spare wrap.
