4 year old still in pull ups and toilet training causing toilet issues. What can I do?!

I am really struggling with my 4 year olds toilet training (potentially on the spectrum and already completed the referral form for him to start the process). He turned 4 in September and starts reception next year so it is crucial I nip this in the bud asap. I had made a few attempts from when he was 3 years old with help from the teachers at his pre school and it has just been a nightmare each time. He has only ever successfully had 1 wee on the toilet. He does have accidents which is what we want, but he does also have days where he holds his wee in all day and that is worrying. The weeing isn’t too much of an issue as he will get the hang of it eventually, but now what has happened, as he has been in pants all week, he has been holding in his poo all week, he is now extremely constipated and keeps saying his tummy hurts, isn’t eating very well and I just have no idea what to do at this point! It has really affected him and I have put him back in a pull up just so he can go for a poo but he is so constipated now that I don’t think he can actually go.

He also doesn’t take medicine, again with him potentially being on the spectrum these things can be impossible, so anything that will help him poo might be problematic. Suppositories are the only thing we can use but you can’t buy them over the counter . He is very particular with his diet and even hiding medicine in food is tricky.

I feel stuck, I feel like a terrible Mum, I have no idea what to do! I feel like my 15 month old will be potty trained before my 4 year old at this rate.

Give him probiotics for kids highest dose

And lactulose is safe

As a mum of a poo withholder - don’t prioritise potty training over this - withholding is much, much worse than still being a pull up. You have a long time until school. Where does he poo when he does go? If he wants a nappy then I would absolutely give it to him. My DD got so constipated and became terrified of pooing and honestly it was the worst time of our lives. Take him to the GP, they can see how badly backed up he is and give him some movicol which will soften it up. Lactulose didn’t work for us because it forced the poo out without softening so just scared her even more.

@Edenmum2 he goes in his pull up, he has never been in the toilet and I don’t think he will for quite some time. I just don’t understand how toilet training can be this difficult. I have tried every trick going and still no luck! No matter how much me and my husband praise him, reward him, show him, explain to him, still nothing.
The issue I have, if Movicol is given orally, this will not be an option as he will not take it :(

Does he not drink juice at all? You can get plain, unflavoured Movicol. I would agree with a pp, with holding can causes issues down the line (my son is on Movicol long term, docs believe he has stretched out his colon which can take a long time to rectify). Can you ask GP for referral/help regarding the toilet training?

Edenmum2 · 10/10/2026 20:10

Movicol can be hidden in loads of different things, there are lots of options. I know it feels really overwhelming but just put the training to one side for the moment and get him happy to start pooing again. I totally understand how frustrating it is and you really have my sympathies, but a year is a long time to conquer the potty training x

Agree with others , completely forget potty training for now and get the constipation sorted asap.
I was as stressed as you 20 years ago but it all came good in the end .. poo in the toilet happened at 5 years old just in time for school …
honestly just take the stress out …
I look back and am appalled about how much I worried….. my dc just “got it “ as they started to go to school and realised that mummy wouldn’t be there to change them at school.

Please be kind to yourself

Potty training can be very stressful! We went through/ are going through something very similar (our little guy has always struggled with constipation, but it got a lot worse when we were potty training. We’ve been trying for a year and he’s only really got comfortable with doing no. 2 on the potty the last few months and even then it takes some coaxing).

Here’s what helped for us:

  • Daily Movicol to make sure he’s not constipated (he was on it for about a month before we got the first potty poop - we use unflavoured dissolved in juice)
  • Making the potty a fun/ relaxed space (we let him decorate it, read favourite books and watch telly on it)
  • Pure bribery (not sure I’d recommend this as then we had to wean him off the bribery - but promising him an ice cream was what landed the first poop)

I also read through advice on ERIC website. The org has a helpline here: https://eric.org.uk/helpline/

Agree with everyone saying to try and be kind to yourself about it - it’s tough and some kids find it harder than others x

I definitely think you should take a break from potty training for a bit, get the poos sorted and then go back to it when he’s better. You’ve still got almost a whole year to deal with the pull ups so no rush, and certainly not as much of an issue as a constipated DS!
