12wk breastfed baby with possible reflux rejecting evening bottle & struggling with Omeprazole timing—advice? Hi everyone, looking for some advice/reassurance

My 12-week-old is primarily breastfed but has had one evening bottle (EBM or formula) at 8pm since day 9 as she was wanting to almost constantly feed in the evenings. She has a slight tongue tie. For the past 10 days, she has started being very sicky in the mornings and very sicky and unsettled in the evenings, she has also started rejecting the bottle, choking on it, being sick, fussing at the breast in the evenings despite seeming hungry which has led me to be concerned that she may have reflux.

We’ve switched from TT closer to nature bottles to MAM anti-colic bottles and EBM rather than formula and she seemed to take the bottle better when offered mid morning yesterday by my partner (as I was at an appointment) and then again when given last night.

Infant Gaviscon helped reduce the sickness over the weekend but she remained unsettled with acid any time she was sick, that coupled with the faff of administration and concerns about constipation led me to want to try her on omeprazole instead.

GP has prescribed Omeprazole. Since it needs to be given cold/refrigerated on an empty stomach 30 mins before a feed, I'm struggling with the timing. Her stomach is emptiest at night, but her waking is unpredictable and I don't want to shock her with cold liquid or disrupt her sleep. Currently, I give it 30 mins after she wakes for the day (as she often has been laying next to me in bed feeding on and off in the hour or so before she gets up) then wait 30 mins to feed. After 2 days, she is more sicky again (though it seems to bother her less).
Any advice on timing of omeprazole or whether I should have stuck with the gaviscon for longer despite the faff or advice on getting her to take the bottle much appreciated. Also finding it hard to fit in pumping around her feeding.

I would carry on with the omeprazole. It is longer lasting than gaviscon. You’re doing everything right. It’s such a worry having a reflux baby but she will get better and you will start to enjoy her more.

The evening cluster feeds aren't about hunger. It's them processing the day and wanting to be close to you. Het brain is developing and they often have a bit of a melt down in the evenings because everything is so new and requires processing and rewiring. You might actually make the fussiness worse by giving her a bottle. It should settle down by itself within a few weeks, but in the mean time you might have a happier baby if you just cuddle and feed all evening.
