Help needed with 3 week old who’s constantly uncomfortable!

I have a 3 week old LB and he is constantly uncomfy and agitated. He has about 4-5oz of milk. I burp him every oz and even more than that. We have to help him poop with every change as he is very hit and miss and gets backed up easily.
He has infacol with every feed and is on aptamil comfort milk. We have put him on a higher teat as he was falling asleep on the bottle and not drinking enough.
I feed him in an upright position.
I have no idea what else I can do, I don’t see the health visitor due to a very bad experience with them with my last child.
He isn’t sick at all with his bottles and having a dummy can make him worse.

any ideas would be appreciated as I’m exhausted!

Cow’s Milk Protein intolerance?
Lactose intolerance?
Reflux?

Have you sought advice from anyone? Are Health Visitors or baby clinics still a thing?

I think a certain amount of gassiness and discomfort is normal. Has he got plenty of wet and dirty nappies? It could be worth mentioning at his six week check up.

To respond to your comments-
yes we do have health visitors but I had a very bad experience with my last two so have refused to see them.

also yes he does have plenty of wet nappies but he doesn’t poo unless I help him with a cotton bud, he can go three days without pooping. I agree that some level but this is a new level where he doesn’t sleep and this discomfort goes on for 12 hours plus despite the amount of burping etc

@SharM1018 can you make an appointment with your GP and discuss any concerns? It could be that the little one is constipated and its causing the pains?

I am tempted, I have messaged my midwife from this pregnancy as she was lovely and supportive.
