Early 4 month sleep regression / CMPA / Dyschezia / Silent reflux??? HELP!

My 13 week old has never been a terrible sleeper at night. He would normally do 3-4 hour stretches, and up until a few weeks ago was doing some 5/6/7 hour stretches.

For the past week, he has been waking up every 40 mins / 1 hour during the night. He starts off by squirming, drawing his knees to his chest, straightening his legs straight in the air and then thrashing them down to the mattress. He then grunts and starts to cry. Sometimes I can soothe him back to sleep with just a dummy and stroking his cheek - again he’ll only last a little while before repeating this. Sometimes he will only settle if I lift him out and hold him and he will settle quite quickly when I do this. Other times he will let out a fart and then that settles him for another little while.

My question is - could this be early 3/4 month sleep regression, and because he’s in lighter sleep he’s more aware of discomfort from immature gut / trapped wind when he’s between cycles? Or does it sound like CMPA making him uncomfortable at night when he’s on his back? He is predominantly breastfed with a bottle of formula in the evenings. I have attached a picture of his most recent nappy for context (sorry for graphic pic!). Sometimes his nappies are quite runny too, but I don’t think they contain mucus (although I’m not 100% sure).

He has laryngomalacia which makes him more prone to silent reflux as he can be quite unsettled and arch his back quite, draw knees in and go stiff during feeds. He started omeprazole 3 days for this and so far haven’t noticed any improvement.

If it’s early 4 month sleep regression (which is looks like lots of people on here go through a few weeks early too) I would be interested to hear of any similar stories - and please tell me it gets better! The hour wakings are absolutely brutal!!
