10m old refusing bottles and food

I’m going out of my mind with worry. My DD 10 months has never been the best feeder. Formula fed since birth as prem. We’ve been through bottle aversion and things were looking up until weaning which she still hasn’t taken to. There was a point we got moving with purées and yoghurt but right now she’s been refusing all bottles and food. She’s been tracking the 9th centile since birth but has dropped to around the 5th in the last few weeks. She started nursery 4 weeks ago and has had back to back illness but seems better now. It could be teething but I’ve tried nurofen, calpol, gels and powders and nothing makes a difference. I’m at my wits end, she’s losing a bit of weight and I’m scared she’s going to end up tube fed because I’m just not getting anywhere. Can anyone help or has anyone had a similar situation that can give me reassurance?
