Help with my 3.5 year old!!!

Hello im after some advice please.. my 3.5 year old is really destructive. background he has had hearing checked due to.covering his ears with sudden loud noises however that was fine and is developing as he should be. He throws things, runs around like an out of control ape 😂 to the point he always hits his head as he flips around on the sofa or runs into door frames. He jumps on people and kicks them. We've tried time oit but he just finds it funny so he now has time out or thinking time in his room. His 11 year old sister is AUDHD. His road sense has got slightly better but he still sometimes runs off and when asked to stop he doesn't listen. It's like he can't hear me or his name and is the same when I try give him instructions im not sure if this is a selective hearing thing. I've spoken to.pre school and apart from covering his ears at times they dont see much of the behaviour that I do. Any advice is welcome.. from one overly exhausted Mum

They dont have the same Father so im completely unsure. It has crossed my mind perhaps he is neurodiverse but that's why im asking advice because I'm stuck on what to do.. wether it could be due to possible traits or just a typical boy toddler

Is he outside a lot? Like I mean, at least four hours a day, running around?

Not like most 3.5 year olds and he isnt a toddler anymore he's nearly school age.
