A bit of background, 4 Yr old came home last Sunday from dad's, gunky knee, cleaned, washed and dressed. Monday it popped, went to MIU confirmed he had something stuck in it. They used Duoderm and told us to come back Thursday. Forward to Thursday, they have confirmed everything is out and it should all scab over and be fine etc. However since Friday morning, these spots have started to appear. Some blister like, some puss filled. Its gotten warm over the last 24hr also. He complains it stings, I've been cleaning with a warm flannel etc. Hes currently sound asleep for the first time in days, so don't wanna disturb if I dont have to. The green line is so I can monitor whether its spreading also. The other imagine is thursday evening after the dressing came off.

has he been given any antibiotics?

if he’s asleep, no temperature and not distressed I’d let him sleep and assess in the morning.

when he wakes up (and whilst you are calling to make a doc’s appointment pop him to play in the bath and add a load of salt to it to again help clean the wound

Ah, they say they are under review! I hope they pop up soon. He was given no antibiotics, no temp, and is happily asleep. They gave it a clean and dressed it at MIU on Monday and it seemed all clear on thursday but has just progressed to getting worse come tonight.

Around the wound, so hopefully the images will load soon but the wound was a small maybe 1p sized in the centre of his knee. The spots/blisters are now all around his knee. There is a good 10+ of them. When I say popped, Im referring to what I think was pressure. So when he came home from dad's, it was raised and full of puss, the wound that had something it it, I took the dressing off and moved his leg and the puss starting dribbling down his leg. Hence why I then took him straight to MIU :)

No, the assumed it was a stone/gravel of some sort? When they took the dressing off, it looked like there was nothing in it, so they assumed it'd come out and it has started scabbing over.

Keep an eye on it from the line you drew till when you go to sleep and if you wake in the night. If its spread then go up. My 18 yo, at the time, we done the same with, with a wound on her leg but she started feeling ill as we seen it spread. She was rushed straight through and put on iv antibtiocs straight away and kept in. I'd possibly be inclined to call 111 for advice. It does sound infected again and likely needs stronger antibtiocs possibly iv. I honestly never though a superficial skin wound would become septic as quickly as it did with my daughter, but it does and he's only little.

Doesn't seem to be getting any bigger, which is good. I drew the line as I noticed it had gone round the side of his knee a little bit, but no more since around 6pm. Its sore to touch, even in his sleep and I was checking the temp of his knee. Hes okay in himself, stops and gets upset occasionally about it being sore. Is having to have regular pain relief, if we don't we notice he complains a lot more about it.

Did anyone at the hospital see anything on either visit. Was there anything stuck on the duoderm.

I ticked the 'sensitive image' box but seems to still not showing, frustratingly. Hopefully it'll appear soon. No they couldn't see anything, well the saw a little black bit in the middle of his knee, but they didn't prod to much as it was too sore. Nothing on the duoderm, was clean when it came off but the wound looked clear.

I think he'll be fine until morning if he was well in himself with no fever. Phone 111 first thing though.

Could be impetigo which can get into an open wound, it's very common but highly contagious so try to stop him touching it and spreading it. I'd call 111 and get an out of hours GP appointment tomorrow.

I'd call 111 and see what they say. They can maybe pass the photos to someone qualified to have a look at/give you an appointment for tomorrow.

It doesn't look like standard impetigo rash, its raised white heads and blisters. Hes also not been touching it cause its too sore, bless him. Got to the point where he's avoiding bending/leaning on the leg.

Is it an adhesive dressing? Wondering if his skin could be reacting to the adhesive? My skin does this and it can be very itchy and uncomfortable and red. Perhaps I could cause little blisters too? If it's that it will follow the pattern of the edge of the dressing.

It was a sticky one, yeah. The dressing was lengthways across his knee but the soreness is all across his knee, so its going past where the dressing was on.
