Evidence for sure start maternity grant

Hello, wondered if anyone can advise from experience —
I’m going to apply for the sure start maternity grant at the 11th hour (5 month baby) as I have recently applied for universal credit to help with maternity leave.
My query is about the evidence. Is it ok to provide the MATB1 form instead of a written statement if the baby is already born? It would be much easier if so. I know it can be accepted but the website isn’t clear whether that is just for applying in pregnancy
Many thanks!!

I used the matb1 form (advised by midwife)
