Can anyone tell me how to get hold of these please? I don't know if this is the right please to post this thread.

I think you can ask your Gp to request them on your behalf from the hospital. You should also be able to write to the hospital requesting them. Some one did give the full details of how a while ago, try searching under childbirth topic.

I phoned my local hospitals PALS service today and a coordinator is ringing me back tomorrow to arrange a meeting to go through my notes.

I rang the hospital - they sent through a standard request form - we had to pay (£15 I think) although I think that varies by location

Phone your hospital and ask for the Medical Records dept. If its anything like my hospital, hey sent me a form to fill in and I sent it back with a cheque for £10 I think it was.
