3.5 yr old won't poo on toilet

My friend's Ds is 3.5yrs. He has been out of nappies for wees for 6 months now and is also dry at night. Problem is he will only do a poo in a nappy and normally only in his room on his own and it can take up to half an hour! (Typical male I say!)
Nightmare for her when she is out, because even though he wants to poo, he holds it in until they are home, and although she has tried everything he refuses to do it on the potty or toilet.
Driving her crazy and she is also worried about the fact he is holding it in. Any ideas?

my nearly 5 year old hates pooing on the toilet if no trainer seat - afraid he'll fall in I think. I have to support him (whiffy!) while he's on the toilet, although he is gradually realising that he won't fall in as long as he concentrates.

Maybe try short periods of sitting on trainer seat to show that it's quite safe?
