Bum rash in 3yo - tried everything!! Is it allergies ?

My poor just turned 3yo ds has had a tough time with rashes recently. He has always had a bit of eczema on his pubic bone which we have finally got a good cream for, but now he is suffering a very persistent anal rash. Its a red raw ring just around his anus which we have tried steroid creams for, anti fungal creams, antibiotics to rule out perianal strep, metanium, sudocream, vaseline - everything 😭 tried different nappies, different wipes. Even tried potty training which lasted 3 days to no improvement and he isnt ready yet so we have gone back to nappies. Got our 3rd gp appointment to try more treatments but i wondered if anyone had similar experience? The rash is red, circular and sometimes branches out a bit with a few red spots, sometimes the skin has broken but hasnt bled. On pooping, he is OK whilst in the act, but immediately afterwards he is crying in pain and his bum cheeks flare up in a red widespread rash that is super sensitive to touch ( sometimes his scrotum too) im starting to wonder if its something allergy related that he is eating? But i didnt know if allergy or intolerance can spring up randomly at his age - and he also doesn't have any other symptoms of allergy. Im rambling a bit but im just so lost as how to help him - its horrible seeing him so uncomfortable.

Sorry I don't have experience of this, but my friends daughter had really bad rashes around her underwear area and it was urticaria.

update: we had our 3rd appt who finally swabbed ds and it came back positive for strep a. So we did a ten day penicillin course. That finished 3 days ago and today the area is bright red with broken skin again 😭 he has pooped a few times today and each time his whole nappy area flares up. He is wailing in pain at changes, refusing baths completely saying they hurt ( i am still flannel washing him !) dropped off stool sample today, but if nothing on that going to push for referral to dermatology

Push a referral.

DS had horrible horrible eczema as a baby. Doctors did an urgent referral as it was rhe worst she had ever seen....

We got a call 1 YEAR later. By which point he had basically outgrown it. The wait times are so so long. Push the doctor to at least get on the list.
