Poo phobia or something else ?

First time poster- Really hoping for some advice asap .
My dd has been potty trained for a few months now and a week ago when she was at nursery I recieved a call to collect her as she had diarrhoea. I collected her right away and it was bad she was hysterical it was just uncontrollable and they had put a nappy on her to try and control " the mess" they said. I was shocked they had done this i had tears in my eyes from her distress.
Now my dd is well again she is utterly terrified of pooing on the potty / toilet so she is holding it untill she cant and soils herself instead. She says it hurts her to go although the stools are soft. She eats alot of fibre , fruits and veg.

The doctors had us send off a sample but advised it could not be constipation as the stools are soft it has to be fear. But why would they ask for this sample if they werent concerned ? Hv have said drs should of offered a stool softener incase she has a hard stool stuck.( I asked but drs disagreed and would not prescribe movicol as hv advised ). Has anyone been in this situation before with their children ? Any advice on how I can ease this fear/ anxiety?
Ive feel ive tried everything , taking her with me , regularly sitting on the potty / toilet, watching videos to distract her ...even a touch of bribery . Nothing is helping and drs arent helpful. Please anyone can you offer any advice ?
