4 y/o won’t poop

Our 4 y/o girl has struggled with poops since potty training began a few years ago. For 6 months earlier this year, we got to a good place where she would go poop in the potty, but since October she has significantly regressed.

She is now at the point where she holds poops for 7-14 days and they eventually come out while she’s sleeping. She won’t even try to go now because she is scared it will hurt. Recently, she also refuses to pee almost all day whenever a poop is near.

As a parent, it kills me because she will often choose not to partake in her normal activities (gymnastics, playing with neighborhood kids, etc) because she wants to just lay around when she has to poop.

We have tried everything including:

-Miralax
-Meeting with the doctor
-Offering gifts and rewards
-Fiber heavy diet
-Sticker charts

She just cannot mentally get past the fear of going poop on the potty and it hurting.

Does anyone have any suggestions?

If she's going that long between pooing, has she had a disimpaction regime of the macrogol? I'd then follow it with a maintenance dose of macrogol to keep the poos soft and pain free, but possibly add in a low dose of a prokinetic laxative (senna or picolax) to encourage her to pass the stool. Together with toilet sitting after meals. I presume you've read the stuff on www.eric.org.uk ?
