Teething - second set of molars - 18 month old really struggling

Hello DS is having a terrible time with his second set of molars. To the point where we’ve been getting calls from nursery because he’s really emotional and not joining in (which is really not like him). I’m getting a GP appointment so he can be checked and have proper pain medication in place as nursery is pretty prudent on calpol unless there’s a drs note, but just wondering if anyone else has had similar with second molars?
The thing that’s making it really difficult is it seems to very up and down. I’ve had a call from nursery today about him being upset and yet he was fine this morning (although he had a bad night after nursery yesterday). We also collected him early last week and he was fine once he was home.
We’ve got my mum staying from overseas at the moment on an extended visit and he’s getting lots of love and attention from her so I’m wondering if there’s a bit of separation anxiety involved too.

Any thoughts or experience gratefully received!

Yeah, the molars can be rough going. Have you tried anything like anbesol? (The liquid is easier to apply, although we use the gel at night, because the gel is harder to spill.) When necessary, we've used anbesol + calpol or nurofen, and the nursery might be more willing to give anbesol, as it's just topical.
