4 year old shouting a night

Hi all at our wits end. We've a 4 year old who's never been a great sleeper. Shouts the house down for us anytime between 11 to 5am. We have to hold their hand to sleep. Bedtime takes over an hour most nights. We have tries sleep training (disaster), reasoning with them, reward charts, getting shouty ourselves nothing works only to give in sleep on a mattress on the floor and hold their hand. My husband and I take turns to overnight with our child it means we don't sleep together anymore It also means one of us is tired. But the shouting wakes us all probably our next door neighbour aswell. We are both working full time and tired. I have tried to explain to our 4 year old that it's not fair or nice to shout at nighttime mummy and daddy need sleep too and it makes us cranky 😭 I'm desperate!
I am tempted now to pay some sort of sleep expert? Or should we just give in to cosleeping til the child is 16? I think there was maybe 3 months when the child was a baby where we got decent sleep in 4 years. Getting feed up with no patience left

Maybe it’s time to see a doctor. They might prescribe melatonin.
