Dd (7) hates doing anything that could be considered "school work"

I wasn't really sure how to phrase my title... DD is 7 and just finishing year 2.

She is a bright, funny, kid who teachers all like. She's kind, and caring, Goes to school without issue.
However, when it comes to doing anything that may take a bit of effort she basically turns her nose up at it, doesn't listen, strops off, gets distracted. Doesn't want to do it basically.

She can read fairly well but will read the bare minimum to us each night.
We have been letting her stay up a bit later to "read" at bedtime.... just means she pulls all her books out, flicks through some of them but never actually reads them.

Maths she is well behind and we are trying to help her at home but it's met with no effort on her part at all.
Simple things she says she can't do, doesn't understand and I think some of it is true but even when we try to explain she doesn't listen.
We even had a tutor to close to a year and it made very little difference.

How do we get her to understand she needs to apply herself a bit and make an effort?

It's the same with hobby type clubs... soon as she might find something a bit difficult, can't do it straight away she just gives up!
Her attention span is awful which I know doesn't help! She can't even sit through a movie!
Really worried that when it comes to year 3 she's gonna start getting even further behind.
