Developmental HV review

Hi just got a letter through for ds 8-12 month HV review. It's in April when ds will be 11 and a half months. Am assuming I don't fill in the questionnaire that says 11 months to 12 months 30 days until just before the review? One of the questionnaires sent is ages and stages and it says 9 months to 14 months 30 days. Do I not fill in this one until 15 months or do I fill this one in and bring it to? I'm very confused I'm a ftm so am not sure at all. Thanks x

Hi, thats different to what we had, ours was 10 months to 12 months 30 days I think. But just wanted to say that our HV said if we wanted to leave it and fill it out at the appointment that was fine so I’m sure yours will be the same. I’d leave the second one and do the first one just before the appointment
