Three year old INCREDIBLY fussy

My DD is 3.5 and was a dream to feed until about 18 months/ 2. Since then she has dropped so many food there's virtually nothing left. For the past few months all she will eat is (and some days she refuses these things):

Weetabix
Milk
Porridge
Bread
Butter
Toddler crisps (one variety only)
Toddler biscuit (one variety only)
Cheese
Corn on the cob
Apple juice (no other juices)
Cream cheese
Oatcakes
Rice cakes
Rice
Chocolate/ ice cream
Sometimes a fromage frais
Greek yoghurt
Two different dried fruit snacks
Pasta (plain)

She is at nursery three days a week and refuses to eat all day so now they've agreed to give her milk at mealtimes so she isn't too hungry.

Eating out is impossible. I sit with her at mealtimes and give her a selection of things including "safe foods" but this has been going on for so long I don't see it as a phase. Can anyone give me any hope?

(She's healthy with loads of energy.)
