21 Month old not talking or understanding!

HELP!!

literally sick of googling things for my little boy whom I have several worries about…
He is 21 month old and has only just started saying mama he doesn’t use this in the correct context and will just walk around saying it, he doesn’t speak any other words nor does he appear to understand any kind of instruction I give him. He will answer to his name and give eye contact when it suits him, his behaviour is very challenging and he knows how to throw a tantrum, as a first time mum I’m struggling with how to deal with these as mentioned before he doesn’t seem to understand anything I say it’s asif I’m speaking a different language!
he will kickoff when he doesn’t get his own way, he will not walk with me while out his pram instead runs in the opposite direction then laughs when I chase him asif it’s a game which results to him been put back in his pram which he hates and will throw a tantrum, I’m not sure if he’s just behind with his development and has learnt to get his own way or if there is more to it I.e autism/ADHD
I have spoke with both hv and doctor who has told me to wait till he has his 2 year review which is in July,
im due my second baby soon and the thought scares me to death with how my little boy is behaving

Just thought I'd add a message of support. I understand how terrible this time is and am going through similar struggles. I would reach out privately to an autism specialist or/and SALT and see what they say. How is your little one now?
