4 month old struggling to do (runny) poo?

My 4 month old has been struggling more and more with his pooing recently. When he was a newborn he didn't seem to have any problems and he pooed a lot. Now he's a bit older he seems to find it increasingly difficult and painful. Every time I tried to feed him today he kept coming on and off and really really crying (like he was in a lot pain) he seemed like he wanted to feed but couldn't keep going. the crying was so extreme at one point I thought maybe I should ring the hospital (I'm living abroad at the moment so no 111) He had a temperature a couple of days ago so I was thinking maybe he's still feeling ill or has a sore throat and feeding was hurting him but then he did an absolutely huge very runny poo and was then perfectly happy for the rest of the evening, so I'm assuming its some part of the pooing process that's causing him issues. He also only poos every other day or every few days. Far less than when he was smaller, and he also only seems to poo when he's feeding (and usually the feeding is very disrupted with crying and squirming) Is this normal at this age? He is exclusively breastfed. I hate seeing him so upset!

Could it be gas? Are you burping lots?

Or maybe a little tummy bug if it doesn’t last long?
