DD struggles to poo - most distressing!

My 2yo dd suffers with constipation (of a sort). She often doesn't go for a few days but then starts to get terribly distressed; lying on the floor moaning and panting, sometimes even crying. She eats loads of fruit, plenty of veg and drinks lots, so her diet is n't really the problem. When she does poo, it is normally soft and not hard little pellets like you'd expect from a constipated child.
I asked the health v., she said it sounds as if she has poor muscle tone in her bowel, but couldn't offer any real help.
She is still in nappies BTW
