5 year old can't make a single decision anymore. Anxiety?!

About 6 months ago my DS (then 4) stopped being able to make decisions. I'd ask him normal stuff during the day in chatting just like usual (would you like toast or cereal? Hmm do you think we'll need our raincoats? Etc) and he just suddenly started to say he didn't know, or would shrug and hold up his hands.

I thought it was a bit odd, so just made light of it and thought it might pass, but 6 months later he is still doing it. He has a scared rabbit look in his eyes like he's overwhelmed by the question, however innocuous.

Has anyone experienced anything similar? Could it be a sign of anxiety or is just a stage? I've pretty much laid off asking him anything so he doesn't feel under pressure but sometimes one will just slip out (last night: would you like some ketchup? Cue: fearful shrugging)

Any ideas?
