Breastfeeding and formula making baby sickh

Hi all, my son is 3 weeks old and has been completely breastfed. I wanted to start combi feeding and gave him an oz of formula on Wednesday. He took it fine but when trying again he was sick through his nose which made him struggle/gasp for breath and was crying hysterically. We went up to the hospital and he was monitored. When coming home I stopped trying the formula and he has been absolutely fine since. I tried again today with 1 oz and the same thing happened. Iam not sure whether to try a different formula or discuss this with the doctor as it could be down to something else?
Any advice or similar situations would be appreciated

Could be a dairy allergy or could be eg flow of teat is too high ( need slow release valve for bottle), could be shape of the teat?
