Personal Child Health Record?

I've put in a claim for UC and it's asking me for a Personal Child Health Record but I haven't been given one and not sure where it comes from?

Have you not been given your red book? It’s usually given when you’re discharged from hospital

I was given my children's red books at the first home visit by the community midwife.

They're not doing home visits in my area due to covid. I had my 5 day appointment which I had to go to hospital for but wasn't given anything.

I didn't gey my red book until the health visitor come for her visit with both my kids. think she come at around three weeks

I was given mine in hospital when I was discharged, probably different depending on area. Maybe give your health visitor a call to ask for it

Ok so I was given the red book today at 10 day appointment but the pages I need aren't filled in...

Call your HV and ask if they can fill it in for you for UC purposes
