5yr old DS not making it to the toilet on time

DS has been dry night and day since about 3, apart from the odd accidents. However, he’ll go through phases where he’ll forget to go to the loo in time and either wee or poo himself. Lately this is happening nearly every day at least once. He says his willy/bottom don’t tell him early enough. He quite often has a loose poo, which makes me think he has actually been caught short rather than just ignoring the need. We’ve tried sticker charts, warnings etc but no luck so far. Any ideas/experience anyone could share?

My 5 yr old was doing this a bit but not a full on accident, more leaving smears in his pants, all very yuck. We came up with a reward system where if on a day he went to the loo on time AND had clean pants come bath time he got a pasta in the jar. When he had 5 pastas he got something nice, one time a pack of Top Trumps, another a magazine eyc eyc - so bigger treats than a packet of sweets. After a few weeks he didn’t need the reward system anymore as it was like he had been reset and was able to go himself in time.
