I need CCLD help please?

DH is doing his NVQ3 in CCLD

He needs details of the immunisations children have

Plus details of 'Health surveillance' in children in our area (Norfolk)

Any ideas where I could find either?

local doctor surgery? ours (Suffolk) have leaflets in the waiting room on both subjects
