18 month happy then screaming

Is there any reason to be concerned when an 18 month old is happy and clapping then begins screaming with tears within seconds? This has happened 3 times and its hard to understand what's going on.

Are they in pain? Could be something like a urine infection. It could be hurting when they do a wee. Or ear infection etc?

He has a little cold. He has a history of ear infections. He isn't pulling at his ears. I'll take him back to the dr just in case. Thanks!
