DD crying at nursery and not being comforted?

My DD aged 1 is settling into nursery. She's done a few half days and seems to have enjoyed them so far. Today she was doing an afternoon and when I went to collect her she was say just in a nappy on the floor crying hysterically with no adult dealing with her. I went in and picked her up and she stopped sobbing and her key worker said she had been happy the whole time but just crying because she didn't want to get changed.

I'm not sure what to think. I want to give the nursery the benefit of the doubt but the key worker was changing another child when I went in so not even attempting to put DD in her clothes? They did reassure me that was the only time she had got upset and that she has settled well but she was very very upset when I first saw her. The nursery is rated outstanding. Would this bother you or is it just a case of these things happen?

I’d give benefit of the doubt to be honest but see if it happens again.
How many babies to adults are there? It could be that a baby needed more attention than your DD at the time? And that she was just having a bit of a tantrum?

Three babies to one adult is the policy but they don't have a full room at the moment so it's more like two babies to every adult.
