4 month old changing from breast to bottle. Won’t drink much!

Hi,
My baby is 4 months old and has silent reflux. She used to scream the house down all day long in pain but since medication from the doctor she is much happier and more settled. She dropped one percentile at 1 month but since has followed the line below which health visitor says in ok weigh gain.
However, she was always really fussy with feeding, refusing the breast and coming on and off all the time. 2 weeks ago I stopped breastfeeding her and moved the the bottle as she would scream when placed on the breast and refuse to feed. It got to a point where I felt I couldn’t go out as she wouldn’t feed and would just be miserable. She will now take the bottle but is the same as the breast in that she will just refuse to feed! She’ll often take 1-2 Oz then refuse any more. Sometimes she’ll drink 4-5 Oz but I’d say that’s once a day max. I think she averages about 12-18 Oz per day plus 2 small breast feeds in the night although I don’t think she gets much from these anymore. It’s such a lot less than the 30oz recommended for her age but I can’t get any more in her! She’s agitated and windy during a feed, pulling her knees up all the time and I give her infacol before each feed. Any tips greatly appreciated!

I should have added she’s on aptamil first milk and has lots of wet and 1 dirty nappy a day.
