Worried about DS 1yo - help!

My DS has had a high temp since yesterday, he spent the day with his grandma but he wasn't himself, would barely sleep and was very tearful. Temp was around 38.

Today he is home with DH and he won't stop crying (wailing really loudly) he's boiling hot between 38.5-39 despite calpol and ibuprofen being given (staggered) and all he wants to do is be held. He's currently asleep on DH but cried himself to hysterics before dropping off.

The exact same thing as this happened last week and it was because he was constipated - he managed to do a huge poo and was fine again afterwards. He's on movicol daily and lactolose and he has done 2-3 tiny poos in the last few days but I have a feeling its the same thing.

I'm at work, but I've called and made an urgent docs appointment - 3.20pm is all they could offer us. I suppose high temp and crying doesn't sound very urgent but he's in absolute hysterics.

Should I be taking more drastic action?
