17 week old started puking

Today my 17 week old DS appears to be uncomfortable and has been sick a few times. It seems different to the usual milky sick. It's more smelly like regular vomit and he seems to be "working" on something, he's grunting and wriggling around in a way which looks like he's in pain

Any idea what this could be?
