Third hand smoke?

My dd is 5 months. My Mil smokes but always outside never in the house or car, until recently I had assumed she was washing her hands when she came inside. Until the other day I watched her come right in from smoking and picked dd up for a cuddle. What I need to know is how much is this an issue? I'm worried that it's been too long now for us to say anything.
We've had some difficulties with them both overstepping boundaries with her, crying because they hadn't baby sat at 6 weeks and we don't see them enough etc. We're just getting sorted with that and now I'm worrying about the smoke 😔

My son has lung/liver issue and we brought this up to his specialist, we were told “if you can smell it, then it’s a problem” doesn’t matter if it’s second or 900th hand smoke, if you’re smelling it then it can affect you
