How to claim child benefit

So I have recently given birth and been discharged with dc2, and just realised the bounty people never came round to me and so I never got my newborn pack.
Having a quick look online it looks like I need a form to fill out to claim child benefit and can't do it it online, and these forms usually come in the bounty packs.

How do I get hold of a form?

If you contact your healthcare professional at your GP surgery they can get a newborn pack for you.
