12 month injection and Hepatitis B

Can any advise what the 12-month injections cover on the NHS, also is Hepatitis B vaccination given from birth, looking at the injection sheet it would appear not to of been given?

HepB is part of the 6in1 injection given at 8,12, 16 weeks

The injection is called Infanrix hexa (DtaP/IPV/Hib/HepB)
