Myself and her dad separated a couple of years ago and I want to go back to my maiden name. I have changed it back by deed poll but not sure how to do about it with the resister office. The Gov.uk website isn't clear.

You can't change that info now. Once the birth certificate have been issued, you've a year to change first & second name then it's permanent.
