you don't have a consent form. I mean, I could fill in the survey for you, but no way you can collect/use the data without a consent form & ethics approval.

You might have ethical approval but where is your consent form? Where is your explanation? Your ethics board won't be happy with you using information where people haven't been given an opt out.

You are definitely missing the ethics form and how people can withdraw from the study. It is normal to include the contact details of your supervisor. I can't even see your name or university name.

People have to give informed consent - so they need to know the aims of your study (hypothesis etc) and how their information might be used (so stating all results are anonymised etc). There needs to be an opt in/out at this point, and you need to provide contact details for any questions about your survey/research. It's just a one page bit at the beginning of the survey - your supervisor and teaching should lay all this out really. Good luck
