Ds is 4.5 and can't do them, but then again has had no need to learn as he doesn't wear shirts and pulls his trousers up and down without undoing the button.
Should I be teaching him? Is he woefully behind?

No, when I was teaching there were lots of 5 yr olds who couldn't do up their buttons easily. In fact, there were quite a lot of 8 yr old who struggle with this!

As long as his other fine motor skills are ok, I wouldn't worry. We all have problems with buttons at times!

It might be an idea to give him a bit of practice though.

DS2 has just started doing up his buttons and he is 4 1/2.
