2 year progress check

I'm a SAHM and DS is approaching 2 years old, how do I get his two year progress check done? Will someone contact me?

The health visitor sent me a letter just after my DC turned two.

It let me know the 2 year check was due, and asked me to phone and arrange an appointment.

Ours was done at 2 years and 3 months, so don't panic if the letter doesn't turn up on DS' birthday.
