Toilet training in reverse

My babe is 38 months and has been potty trained since september 2006. Every now and again (about once every 4-6 weeks) she has quite a few accidents in a few days. In general she has great difficulty tearing herself away from her play resulting in damp knickers on a fairly regular basis. She's also a twin whose sister is also potty trained but doesn't have any accidents. We don't make a big deal about this though.
I've just come to type having cleaned up a big sh*t in the living room, the second in two days! Not accidents, in fact almost everytime she's needed the loo in the last four days she's just wet herself where she stood. I've talked with her about why (got nowhere), taken away treats, made her wear trousers (she's going through a trouser hating phase), shouted at her (I'm not proud of it),cuddled her, kissed her and now I am silent as I just don't know why it's happening or how I should react. The only other thing that I can think may be somehow relevant is that we returned from a long haul holiday 4 days ago and things went 'down the pan' from then. Any advice? Has this happened to anyone else? Am I being a big doofus and missing the glaringly obvious reason for her behaviour? What should I do?
