Memory/family tree book recommendations?

I am looking for a book for a bright nearly 10 year old to record details of her terminally ill father's life. Ideally I would like something structured for her to fill in (have considered just a blank notebook and some heading suggestions but I think she's a bit young for that). However I would also like something that's a bit flexible so that she can start it by asking him but depending on when he passes away or on her interest level she could continue by asking family and friends. I did have a look on amazon but they all seemed to be focused on after the person had died rather than to start beforehand. I don't know whether what I have in mind actually exists but I am open to other ideas - any suggestions much appreciated, thanks.

I got one for us and for my mum from Priceless Treasures. The books are meant to be filled in by the adult but it doesn't really matter. The point is it has sections with headings like 'where did you go to school' 'what was your best friend's name' 'did you have a pet when you were young?''where did you and mum meet' etc and you can get it personalised by sending the company photos which they will add to the cover.
