Irritation on baby's bottom

My dd wears disposable nappies and only recently, she's started scratching really badly, every time we change her nappy. She always scratches in the same place, top of the bottom/lower back area. She scratches to the point of actually ripping the skin. She doesn't attempt to scratch during the day-just when we change her nappy.

Has anyone else experienced this. My dd is 15 months so I find it strange it's suddenly started now. What could be the cause??? Please help, she looks like she's actually hurting herself with the scratching!!!
