Tax credit form help

does anyone have or know anyone who has a spare form for 2011/12
i am in a desperate situation been waiting for one from them for last 2weeks still no sign and now can take another 2weeks to arrive

please please help

They're a bugger but worth it if you think you're eligible.

Thanks its for a new claim and they only will post them its been 3 weeks now still not had it,

Phoned up like 12 times now they will only send out 2nd class each time and say it will take upto 2 weeks

not good at all

Keep calling the HMRC Tax Credit Helpline. 0345 300 3900 Call them daily until it arrives. Make a nuisance of yourself.
