Name change

I want to change my dd middle name. Has anyone done this? How do I go about it?

How old is she?

We changed my ds's middle name at about 4 months old.

We phoned registry office who sent us a form, we filled it in and sent it back and they amended his birth certificate to show his new name.

Then it was just a case of informing all the relevant people. Doctors etc.

If your dd over 12 months you will need to do it by deed poll which is very easy and there's plenty of sites where you can do it over the Internet now.

Sorry, they didn't amend it, they added his "new" name to it.
