Teaching children about personal safety

I've been reading a book about talking to children about personal safety and teaching them how to avoid and get out of bad situations. One point the quthor makes is that, if something bad happens, particularly if it is sexual, the child must not be made to feel responsible - it is never their fault.

What bothers me is, how do you teach cause-and-effect, so that a child will (eventually) learn to be responsible for their own actions?
