Worried about smoking and newborn baby

Hi there, I am a bit worried about my new neice - she is 3 weeks old and when I first held her, I thought she smelled a bit smoky. Then when I was round there the other day my sister was at the back door having a cigarette.
I thought she had given up, but obviously not. I don't have an issue with her smoking as it is her choice to do so, but she straight away went and picked up her daughter and was cuddling her, sticking her knuckle in her mouth to see if she was hungry etc.
To make matters worse, my neice was diagnosed with a heart condition when she was born.
I am really worried about this, but I don't want to interfere.

Should I say something?

yanbu, but if you sister doesnt care enough about her daughter to protect her health then theres nothing you can do Sad for that poor baby though.

I know - I guess I was just wondering if I should mention the fact that even the toxins on her hands are harmful - I know it sounds silly but I'm not sure whether she is aware of it or not.
